Travel Vaccinations
If you require any vaccinations relating to foreign travel, we would ask you to complete a “Travel Questionnaire Form”. These are available online or you can collect one from the surgery. Once completed, a nurse will determine if any vaccinations are needed.
The Nurses will call patients that do not need injections or malaria tablets to give general travel advice.
Please ring into the surgery in 7 days to book a travel appointment if you have not had a call from the nurse.
Some vaccinations can only be done at a travel clinic, and you may be charged for certain ones as they are not available on the NHS.
Please allow at least 6 weeks before your travel date.
Travel Health Questionnaire
To help us offer the appropriate advice, please fill out the online form below before coming to see the nurse.
You can also download the questionnaire to bring in to the practice (PDF, 62KB)
